Permission to Use and License the HFS-II:

The copyright for the HFS-II is owned by the University of Virginia and the survey can only be used with permission. Permission and licensing agreements are managed by HFS-Global LLC. There is typically no licensing fee for students and colleagues who wish to use the survey in their own research or clinic setting. However, there is a licensing fee for use by for-profit organizations, such as pharmaceutical and medical device companies. Contact us for any questions regarding permission to use the survey.
